Program Design Overview
Horizon Hearts Inc. delivers a continuum of care for underserved individuals, with a focus on housing, healing, and empowerment. Our programs are designed to meet immediate needs while fostering long-term stability, personal growth, and community reintegration.
Horizon House
Type: Transitional Shelter for Men
Capacity: Up to 20 adult men
Purpose: Horizon House provides short-term housing and wraparound support for men experiencing homelessness, displacement, or crisis. The program emphasizes stability, accountability, and personal development.
At Horizon Hearts, in collaboration with Damatrix Studios Network Multimedia Arts Program, we offer a precious opportunity to develop each youth unique ability to build skills that benefit them a lifetime. It’s a valuable program resource for both young men and women. This program can also serve as an after school program for school-aged kids before parents return home from work and because very few community supervised resources and activities for schooled-aged kids are available. In this amazing program, our youths learn, but not limited to the following:
Core Services:
- Safe, structured living environment
- Individual case management
- Mental health and substance use referrals
- Life skills workshops (e.g., budgeting, conflict resolution)
- Employment readiness and job placement support
- Reentry support for formerly incarcerated individuals
Program Goals:
- Stabilize housing and health
- Foster independence and self-sufficiency
- Prepare residents for permanent housing or reunification
Horizon Place
Type: Emergency Shelter for Men
Capacity: Up to 32 adult men
Purpose: Horizon Place serves as a low-barrier emergency shelter for men in immediate need of safe housing. It operates with a trauma-informed approach and prioritizes dignity, safety, and rapid access to services.
Core Services:
- Overnight accommodation and meals
- Intake assessment and triage
- Crisis intervention and de-escalation support
- Referrals to medical, mental health, and substance use services
- Pathways to transitional housing (including Horizon House)
- On-site peer support and community engagement
Program Goals:
- Provide immediate relief and stabilization
- Connect clients to longer-term solutions
- Reduce barriers to accessing care and housing
Integrated Support Services:
- Both Horizon House and Horizon Place are supported by Horizon Hearts’ broader service ecosystem, which includes:
- Counseling & Case Management
- Youth Empowerment Initiatives
- Community Education & Advocacy
- Partnerships with local hospitals, studios, and service providers
